My bf got it for me off of Ebay...the only one i've seen on Ebay. It is an NBA Player Bobble Limited Edition Collectible (#ed to 504). The box has the Website www.forevercollectibles.com on it. Maybe that will help you. The Utah pin pictured with the bobblehead (on top of the jersey that Bogut is holding) I got somewhere else. We just put it on there for the picture. Hope you fine one.
